Job Overview

We are seeking a dedicated and compassionate Hybrid In-Home Clinician (Nurse Practitioner or Physician Assistant) to provide high-quality care to complex Medicare patients through a combination of in-home visits and telehealth consultations . In this role, you will conduct comprehensive assessments, identify healthcare needs, and coordinate complex care plans to improve patient outcomes. Key Responsibilities

• Conduct comprehensive in-home and virtual assessments for Medicare Advantage patients, including medical history reviews, medication reconciliation, physical exams, and various health screenings (social determinants of health, mental health, frailty, etc.).
• Identify and document healthcare needs, gaps in care, and risk factors through detailed assessments.
• Develop personalized care plans and coordinate with primary care providers and specialists as needed.
• Educate patients and caregivers on health conditions, medications, and self-management strategies to promote better health outcomes.
• Collaborate with a multidisciplinary team , including registered nurses, social workers, and healthcare providers, to ensure effective care coordination.
• Provide guidance and support to patients and their families, enhancing their quality of life and overall well-being .
• Conduct telehealth visits when appropriate, ensuring continuity of care and timely follow-ups .
• Maintain accurate and up-to-date documentation of assessments, clinical encounters, and quality measures, in compliance with Medicare and Medicaid regulations.

Qualifications

• Valid and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice.
• Experience in geriatrics, primary care, home health, telehealth, or complex care management preferred.
• Strong clinical assessment and diagnostic skills.
• Excellent communication, collaboration, and patient education skills.
• Comfort with telehealth platforms and virtual patient care .
• Ability to work independently while being an integral part of a multidisciplinary healthcare team .
