Job Description

Job Summary

The Care Coordinator at FiftyForward works closely with aging adults across with the Living at Home, Care Team, and Conservatorship programs. Core responsibilities encompass conducting in-person client visits, conducting assessments, coordinating resources, advocating for clients, managing documentation, and aiding in departmental operations.

Job Responsibilities

  1. Administer comprehensive assessments for clients entering our case management programs to determine eligibility, identify needs, and tailor services accordingly.
  2. Develop and implement individualized care plans, modifying them as needed, with attention to current and future needs.
  3. Collaborate with conservatorship program staff to complete annual accountings, client visits, and on-call telephone coverage.
  4. Connect with colleagues, community partners, and governmental agencies to ensure individualized, holistic support for each client.
  5. Submit referrals and applications to link clients to applicable resources.
  6. Respond to resource line inquiries and connect callers with appropriate services.
  7. Incorporate evidence-based practices into client care and decision-making.
  8. Maintain accurate client records and submit all required reports in a timely manner.
  9. Assist with bill payments and maintain accurate records of all transactions.
  10. Provide transportation for clients, as needed.
  11. Support and collaborate with FiftyForward Centers and other internal programs.
  12. Actively identify and maintain strong relationships with community partners.
  13. Conduct outreach events to promote services and increase engagement.
  14. Adhere to the National Association of Social Workers Code of Ethics.
  15. Perform other duties as assigned.

Qualifications

Education:Bachelor’s degree in social work or a related field. Equivalent experience of three years can substitute for formal education.

Preferred Experience:Care management, direct care, legal practice, work related to aging adults, governmental benefits, and/or related social services.

Additional Skills:Strong interpersonal and problem-solving abilities for both independent and collaborative work. Familiarity with trauma-informed care, motivational interviewing, and culturally competent practices. Knowledgeable about Middle Tennessee social service networks. Possess reliable transportation, a clean driving record, and vehicle liability insurance. Bilingual in Spanish, offering additional value.

Certifications and Licenses:Valid driver’s license required. Must have or obtain CPR and First Aid certification within 90 days of hire.

Physical Aspects:Requires travel within Davidson County and surrounding counties. Includes home and facility visits, with potential exposure to odors, animals, and excessive clutter. Must be able to climb stairs, bend, push, pull, stand for extended periods, and handle loads up to 25 pounds.

Equipment Requirements:Computer, printer, copier, and telephone.

Working Hours:Full-time, 40 hours per week. Standard hours are Monday through Friday, 8:00 a.m. to 5:00 p.m., with occasional early mornings, evenings, or weekends as needed.
